A. In the event that Alternate Payee, _________, dies before the receipt of h— benefit, such Alternate Payee's interest shall be paid to _________[name], _________[address], Social Security Number _________, as an Alternate Alternate Payee.

B. Death of Alternate Payee. The Alternate Payee shall be entitled to name a beneficiary (or beneficiaries) so that if —he dies before amounts held under the Plan are paid to h— the named beneficiary (or beneficiaries) will be entitled to receive the amounts held on behalf of the Alternate Payee under the Plan. The death of the Alternate Payee prior to the determination that this Order constitutes a QDRO shall not alter the obligation hereunder to distribute the benefits granted to the Alternate Payee to h— beneficiary. For purposes of this paragraph, the term "beneficiary" shall mean any individual or entity named by the Alternate Payee in a written notice to the Retirement Committee or, in the absence of such notice, h— estate. All distributions to the Alternate Payee's beneficiaries shall be made pursuant to the provisions of Section 401(a)(9) of the Code.

C. In the event of the death of the Alternate Payee prior to the death of the Participant, any and all payments and/or survivor benefits payable to h— by the terms of this Order shall revert completely to the Participant, and the assigns, heirs and/or estate of the Alternate Payee shall have no interest whatsoever in such payments and benefits.
