As a lease was heretofore entered into by undersigned with _________, dated _________, covering premises described as _________, whereby premises were leased to undersigned for one year from first day of _________[year], to _________ day of _________[year], copy of lease being hereto attached; and

As a lease covering above-described premises was heretofore executed and entered into on _________[year], between _________ and _________, whereby possession was taken of premises by _________, now deceased, which possession was continued by undersigned at death of _________; and

As lease above set forth as having been entered into on _________[year], between _________ and undersigned, was entered into by undersigned with understanding that _________ was legal owner of above-described premises; and

As, subsequent to execution of lease notice was given undersigned on or about _________[year], by _________ that _________ was the legal and lawful owner of premises and entitled to immediate possession of same, inasmuch as the relationship of landlord and tenant had never been established between _________ and undersigned, and that undersigned should vacate such premises at once; and

As undersigned, on investigation, found that _________ was the lawful and true owner of premises, and entitled to immediate possession thereof, and undersigned, desirous of remaining in possession of premises and to avoid being ousted from premises by _________, did thereupon acknowledge _________, to be the lawful owner of premises, and acknowledged _________ as landlord, and did not attorn to _________ as tenant of _________, in possession of premises, and entered into a lease for premises with _________; and

As undersigned now holds to be in possession of above-described premises as tenant of _________, and does acknowledge _________ as the true and lawful owner thereof, paying rent to _________ for the use and occupancy of premises,

You are hereby notified that in view of the above and foregoing facts, I have elected to consider lease heretofore executed by me covering premises with _________, dated _________, copy of which is hereto attached, as aforesaid, to be null and void, and of no force and effect and I hereby deny that I am holding premises as tenant of _________, having heretofore elected as aforesaid to recognize _________ my landlord and owner of premises.
